Below are links to some useful Health Services Forms for parents of Visitation BVM School children. These forms and procedures are provided by the Norristown Area School District.
Health services are provided by the Norristown Area School District. The district provides a nurse 1 day a week, and the parish provides a nurse 4 days each week. The Certified School Nurse is responsible for maintaining your child’s school health record and immunization information. In addition to routine first aid, the school nurse dispenses medications if ordered by your child’s physician, performs state mandated vision, hearing, height and weight screenings, and scoliosis screening. In addition, she provides health-related presentations to classrooms.
Physicals are required for entry into school at the kindergarten or 1st grade levels and at the entry to 6th grade.
If your child has sustained a sports injury or has had another situation in which his/her physical needs have changed (even temporarily), please notify the school nurse and provide a physician’s note. Your child’s safety and well-being is the goal of the school nurses.
A district dental hygienist provides dental screening in grades kindergarten, 2, 5, and 8. With parent permission, a weekly fluoride rinse is given in grades 3 and 4.
