Our registration dates for students new to Visitation BVM School have passed, but it is not too late to register for next year.  Please contact the school office at 610-539-6080 to set up an appointment now.
 
 
Registration Dates for 2012-2013 School Year
  
Pre-school (4 yr)
Friday, March 30, 2012 - 9 to 11 AM
Kindergarten
Thursday, March 29, 2012 - 9 to Noon
Grades 1-8 (new students)
Friday, March 30, 2012 - 9 to 11 AM
Re-registration for Grades 1-8
Wednesday, April 11 - April 25
 
Requirements for Registration for Preschool, Kindergarten, and New Students to Grades 1 to 8 for the 2012-2013 School Year
  • The Registration Fee for Pre-School is $100.  Registration fee for Kindergarten - Grade 8 is included in tuition.
  • Original, state-issued Birth Certificate
  • Baptismal Certificate **
  • Immunization / Health Record ***
 
Immunization Requirements by Pennsylvania Law
 
Preschool, Kindergarten, Students Grades
1-8
Diptheria-Tetanus (DTP, DTaP, DT, or Td)
4 or more doses, properly spaced.  Dose #4 to be given on or after the 4th birthday.
 
Polio
3 or more doses, properly spaced.
 
Measles, Mumps (MMR)
2 doses, properly spaced.  Dose #1 given on or after 1st birthday.
 
Rubella (German Measles) (MMR)
1 dose
 
Hepatitis B
3 doses, properly spaced, Dose #2 given 28 days after Dose #1.
 
Varicella (Chicken Pox)
2 doses, Dose #1 given on or after 1st birthday.  Immunity from vaccine or written proof by doctor of Chicken Pox Disease
 
Tuberculosis
1 test per questionnaire response
Students Entering 7th Grade ONLY
Tetanus, Diphtheria, Acellular Pertussis (Tdap) Vaccine
One dose, if 5 years passed since last tentanus immunizations.
 
Meningococcal Conjugate Vaccine (MCV)
One dose, if not given previously.
 
**Baptismal Certificates are provided by the church where your child was baptized.  If you do not have your child’s certificate, please contact their church as soon as possible so you will have the Baptismal Certificate with you on registration day.  This includes children who were baptized at VisitationBVMChurch.
 
***Students will not be able to start school in September 2012 without written proof from their doctor or clinic that they have received the required immunizations.  Written proof must be provided at registration so it can be kept with your child’s health record in the nurse’s office.
